(1) RADIAL NERVE INJURY :-
Wrist drop is also known as radial nerve palsy , is a condition where a person cannot extend the wrist , fingers and thumb .
PHYSIOTHERAPY TREATMENT – Physiotherapy treatment start after wrist drop .if radial nerve is damaged by stretch or compression injury(eg. Road traffic accident, fall from height, etc) ,get good result with the help of physiotherapy treatment . If radial nerve is damaged by sharp cut injury eg. knife , weapon or gun shot injury ,blast injury – need surgical repair or graft or tendon transfer followed by physiotherapy treatment.
(2)ULNAR NERVE INJURY:-
Weakness in the hand or fingers( Ring and Little finger) ,severities known as claw hand. numbness or tingling increased while writing or typing.
PHYSIOTHERAPY TREATMENT – physiotherapy treatment start if ULNAR NERVE is damaged by pressure on the nerve ,arthritis, elbow dislocation, wrist fracture - needs physiotherapy for good result . If nerve is damaged by sharp cut – need surgical repair followed by physiotherapy treatment.
(3) MEDIAN NERVE INJURY:-
Weakness of hand , fingers (index, middle ) and thumb, wasting of short muscles of thumb also known as ape hand deformity , pain , numbness and loss of sensation of index, middle half of the ring finger and thumb . Patient unable to do fine movement eg. writing , brushing teeth, combing , turning knob etc.
(4)CARPAL TUNNEL SYNDROME :-
It is due to compression of median nerve . A comprehensive physiotherapy program for carpal tunnel syndrome will include a combination and sequence of therapies that are designed to help reduce inflammation, pain and weakness or numbness in the hand, wrist and/or arm.
PHYSIOTHERAPY TREATMENT – If nerve is damaged by compression, fracture , stretch - need physiotherapy treatment for good result . If nerve is damaged by sharp cut eg. glass, knife etc need surgical repair followed by physiotherapy treatment.
(5)FLEXOR TENDON INJURY : –
Flexor Tendon Injury divided by different zone in Hand – Zone 1 to Zone 5 , when flexor tendon completely cut , the fingers cannot be bent . A cut tendon need to be repair .
Treatment:- After 2 to 4 day of surgery need to start exercise by the Hand therapist under plaster cast . If only Tendon repair applied KLINERT TRACTION and continue exercise .If nerve repair along with tendon repair then Klinert Traction delay for 2 weeks . Remove plaster partially after 3 to 4 weeks and continue Hand physiotherapy treatment till full closure of Hand .
(6) EXTENSOR TENDON INJURY :-
Extensor tendon can be injured by a minor cut , Laceration , Trauma , overuse etc. It divided into Zone 1 to Zone 8 . When Extensor tendon completely cut ,the fingers cannot be completely streten . A cut tendon need to be repair .
Treatment:- After 1st dressing if wound is good then start exercise within plaster cast and applied klinert traction , change the position of plaster after 3 weeks , and continue exercise till full closure of hand .
(7) BRACHIAL PLEXUS INJURY :-
The Brachial plexus is a group of nerves that come from the spinal cord in the Neck and travel down the Arm . These nerves control the muscles of the Shoulder , Elbow ,Wrist , and Hand , as well as provide sensation in the upper limb . Brachial plexus can be damaged by
(A) Stretching eg. – Motor bike or car accident ,
(B)Pressure eg. – during fracture or dislocation ,
(C) Sharp cut – by knife , gun shot , or
(D) Blast injury .
Treatment:- Experienced Hand Physiotherapy treatment provides good recovery of nerve.
If Brachial Plexus is injured by Stretching or pressure – good recovery is possible by Physiotherapy Treatment ( Conservative Treatment ) .
If Brachial plexus is injured by sharp cut eg. knife , gun shot or blast injury – need to surgical repair or nerve graft or muscles transfer followed by physiotherapy treatment .
(8) VOLKMANN’S ISCHEMIC CONTRACTURE ( VIC ) :-
VIC is a deformity of the Hand , Fingers and Wrist caused by the injury to the Muscles of the forearm . VIC occurs due to compression over blood vessel due to tight plaster after fracture , crush injury , can lead to decrease blood flow to arm .
Treatment:- Start physiotherapy protocol for VIC and regular splinting help to correct deformity of Hand and provide complete closure of fingers .
(9) CRUSH INJURY :-
Crush injury occurs when the limb or body part are subjected to a forceful compression which causes stretching and pressure on these body part due to road traffic accident , roller machine , fall of heavy object on limb . After wound healing affected body part unable to move .
Treatment:- Complete recovery is possible with the help of expert physiotherapy management according to assessment .
(10) POST OP. FRACTURE :-
Most bone fracture are caused by falls and accident .Fracture may also be the result of weakness of bone for eg. osteoporosis , cancer etc. Treatment:- Postoperative physiotherapy treatment is very important in complicated fracture and along with associated tendon injury condition . full function is possible with expert physiotherapy management.
